This event will be looking at the role of communities in health and care. It will examine how to create sustainable, community-based health and care, and share learning from areas where communities are taking a key role in improving and sustaining good health.
Looking at examples of:
- Community development. Where communities are strengthened so that they can improve their own health – or factors that affect their health – without necessarily relying on formal health and care services
- Community commissioning. Where communities have greater control over the commissioning of health and care services and other services that affect their health.
- Communities and service design. Where communities are directly involved in the selection and design of services.
- Community organisations. In pathways of care where the scope of services that the health system is able to provide is expanded, often to include those that support social, rather than medical, models of care.
